If I am changing positions with the same employer, do I need to obtain HPRP approval?

Yes.  Any change in an HPRP participant’s job position, even with the same employer, needs to go through the approval process in team review.  Submit the Monitoring Agreement Change Request explaining the change, whether the Worksite Monitor changes also and obtain providers’ signatures.   The Worksite Monitor will be contacted by the Case Manager to review the changes and presented in team review for a determination.
