My backup worksite monitor no longer works with the company. What should I do?

Contact your case manager as soon as you are aware that either the backup worksite monitor is no longer working with you. Your case manager will contact your worksite monitor to determine who the most appropriate  person is for a backup/emergency contact and make the necessary changes in your chart. You must also submit a release of information for this individual.
