When can I start working if I am not employed when my Monitoring Agreement Begins?

If a licensee is not employed at a HPRP approved and confirmed employer at the time they enter a monitoring agreement, the licensee must complete and submit a “Request to Change their Monitoring Agreement” form to their case manager.  The “Request to Change their Monitoring Agreement” form must have the signatures of the licensee’s providers.  The Licensee must include the name and address of the potential employer and the name and telephone number of the potential worksite monitor.  The licensee should also complete, sign and send a “Release of Information” form for their case manager to speak with the potential worksite monitor so a confirmation can be done by the Case Manager.  Once all the documents are received and the confirmation is done, the Case Manager will present the employment request during team case review.  A LICNESEE SHOULD NOT BEGIN WORKING WITH A POTENTIAL EMPLOYER IN ANY CAPACITY UNTIL AFTER THE EMPLOYER AND WORKSITE MONITOR HAVE BEEN APPROVED BY THE HPRP.
