When do I have to submit a Leave Request?

Leave Requests need to be submitted when a Licensee will be travelling outside of the State of Michigan and sent to the Case Manager  a minimum of two (2) weeks in advance and signed by providers.    Licensees will not be excused from attending 12 Step Support Meetings or testing with FirstLab.  Leave Requests also need to be submitted in the event of an emergency even if advance notice is not possible.  It is the responsibility of the Licensee to confirm that the request has been approved and processed prior to leave except in emergency situations.
